New OASIS-E1 version and PDGM CY2025 Updates

OASIS-E1 Igea and iPOC update

OASIS-E1 is here! Igea HHC and iPOC have been updated with the new version of OASIS assessments and iPOC comprehensive forms.

The following iPOC forms have been added and are now available for any visit or assessment performed 1/1/2025 or after:

  • SOC (E1)
  • PT SOC (E1)
  • Resumption of Care (E1)
  • RECERT (E1)
  • Transfer w/o Discharge (E1)
  • Transfer with Discharge (E1)
  • Discharge (E1)
  • PT Discharge (E1)
  • Death at home (E1)

From Igea’s section Operations / Patient Services / OASIS, any OASIS assessment created with date 1/1/2025 will be created under new version 3.01 OASIS-E1.

We are using Igea’s OASIS-E forms. What should we do to start using OASIS-E1 forms?

Nothing! This update has automatically updated all your current services having OASIS-E forms associated and added the equivalent OASIS-E1 version of the form, discontinuing the D1 version:

We are using our own custom OASIS forms. What should we do to start using OASIS-E1 forms?

Same as with any new version of OASIS, Igea offers the tool to convert your E version of the forms into the E version.

  • Go to Tools / iPOC Designer / Forms Designer
  • Search and select the custom OASIS form under E version you would like to convert to OASIS-E1 version
  • Click the button “Convert To OASIS-E1”

  • The system will convert your custom OASIS-E form into a new equivalent OASIS-E1 version. Your E form will not suffer any changes, a NEW OASIS-E1 version of the form will be created.
    After the process finishes the system will display a pop up message listing which OASIS Questions were removed and which ones added based on the new OASIS version and RFA.

After your new form has been converted it is suggested you review the sections and location of non-OASIS questions and perform the necessary changes (sections ordering, ordering of questions, etc) as needed.

Once you finished converting your custom forms to the new OASIS-E1 version you must associate the new forms to the applicable Services:

  • Go to Settings / Services and Supplies / Services.
  • Search the service currently having the OASIS-E version of your custom form and edit it.
  • Go to tab Point of Care Forms
  • First you must discontinue the OASIS-E version of the form by setting the Effective Through date as 12/31/2024
  • Then add you newly converted OASIS-E1 equivalent form setting the Effective From as 1/1/2025:

PDGM Medicare CY2025 Final Rule & Grouper updates

The system has also been updated with the new rates, amounts and constants for PDGM 2025, as well as the grouper elements, CBSA, Wage Index, HHRG Weights and ICD10 codes PDGM changes.

Beginning July 1, 2025, all Medicare / Medicaid Certified home health agencies will be required to collect and submit OASIS data for all patients, regardless of their payer source. This includes patients with a Start of Care (SOC) M0090 date of July 1, 2025, or later.
